Dr. Jason Lake, MD

Interim Chief Medical Officer

Dr. Jason Lake is a physician executive with extensive experience in clinical leadership, operations alignment, and organizational transformation across diverse healthcare settings. He brings a values-driven, relational leadership approach grounded in clarity, collaboration, and trust. Jason is partnering with Heritage Health to strengthen clinical leadership, provider engagement, and sustainable governance structures.


Jason has held senior leadership roles including Chief Medical Officer and Executive Vice President at Confluence Health, where he led all clinical and behavioral health service lines across a regional health system. He also served as National Medical Director of Primary Care at Walmart Health, where he designed integrated care models aligning primary care, behavioral health, and dental services.


Throughout his career, he has been recognized for bringing clinical and operational teams together around shared purpose, establishing sustainable leadership structures, and navigating complex change with integrity and transparency in service of patients, care teams, and the communities they serve.


Jason earned his medical degree from the State University of New York at Buffalo School of Medicine and completed postgraduate training in internal medicine and gastroenterology at Walter Reed Army Medical Center, where he served as an Army physician for over a decade. He holds a Master of Medical Management from the USC Marshall School of Business and a Master’s degree in Organizational Change and Leadership from the USC Rossier School of Education.
