Assertive Community Treatment

The Assertive Community Treatment (ACT) and Level 5 Team is available for adult individuals with Medicaid who have been diagnosed with a severe and persistent mental illness.

This is an intensive wraparound treatment model that supports individuals in gaining a higher level of stability and working towards independent management of daily life.  Participants should expect multiple weekly visits with treatment team providers.

Participants can expect:

  • Individualized treatment specific to you and your needs. 

  • A team approach, with various specialties

       to support all aspects of your life.

  • Face-to-face contact with a member of the treatment team no less than 4 times weekly.

  • Both in office and in community contact with members of the treatment team.

  • If needed, coordination with payee, family, residential staff, medical providers, etc.

Participants Agree To:

  • Attend individual therapy each week

  • Attend group therapy weekly.

  • Meet with their Heritage Health psychiatric prescribers monthly (or as needed).

  • Work with all members of the ACT/Level 5

       treatment team.

  • Make appointments with treatment team
