Our Diabetes based Shared Medical Appointments are designed to help patients learn how foods affect their blood sugar and how to build the right meal plans for success. Along with diet, a focus is placed on adding activity into your everyday lifestyle and helping you set goals for weight loss.

All patients should first have an individual consultation with their Heritage Health Provider prior to scheduling an SMA.

Diabetes Basics

This course is for patients who are newly diagnosed and are just beginning to learn about diabetes. In these classes, you will find basic and useful information on blood glucose monitoring, meal planning, medicines, and exercise. A strong focus will be placed on developing real-world strategies to manage your diabetes.

Treating Diabetes with Natural Remedies

These classes discuss the role of diet and exercise in maintaining a normal BMI and improving blood sugar levels. Our focus will be on decreasing medications for diabetes, along with goal-setting for weight loss and management of hypertension and cholesterol. We will cover the standards of diabetes care and address preventive care to help avoid diabetic complications.

Kroc Pre-Diabetes

This eight week course is intended for patients who fall into the category of Pre-Diabetes and who also have either High Blood Pressure, High Cholesterol or both and provide an environment that allows patients to work on preventing diabetes.

Diabetes Management

This is a patient-centered, team-based class that helps the patient manage their diabetes/pre-diabetes care by providing essential components from key healthcare professionals. The patients will be able to receive, learn, comprehend, and implement the self-management education that is imperative to control their disease. Goals will be selected by the patient and fit into their daily life in order to succeed. In the end, the patient should be able to identify ways they can live a healthy, productive life without diabetes-related complications.
